Instructions: The online complaint form is a 4 step process - it will allow you to enter your complaint information - review and make changes if desired - and, finally, send your complaint via email to the P.O.A.office.

1. Fill in the form below (the information marked as "Confidential" will be kept for office records only and not shared)
   
   
2. Click the Review button at the bottom of the form which will take you to the Review Page.
   
   
3. The review page will show you what your complaint will look like when it is emailed to the P.O.A. office. If you need/want to make changes, the input form is also on the Review page (pre-filled with your information) so you can just make the required changes there and click the Review button on that page to review your altered complaint.
   
   
4. When you are satisfied with your complaint, simply click the "Ok - Send It" button on the Review page to send your complaint to the P.O.A office -- NOTE: we do NOT store these complaints electronically, so, should you want to print a copy of your complaint for your own records, your complaint will be redisplayed following it's being sent to the office -- you can simply print your browser screen to get a hard copy.
